Basic procedures and lab tests are also included in the monthly fee
If you have insurance, you can still use it for things like labs, medications, and specialist appointments
If you don’t have insurance, you can still receive comprehensive medical care and access to discounted labs

$95 per month
Family discounts will be available at provider discretion for up to 2 adults and 3 minor dependents living in the same household.
2nd family member is $75 per month
3rd, 4th, or 5th family member is $35 for each person per month
The direct primary care membership fee includes up to 12 visits per year focused on weight management. For members who desire more intensive monitoring, there is an additional $25/month fee on top of the monthly membership fee.
The initial visit is $150. After the initial visit, there is a $95 per month fee. This includes any in person visits, texting, and portal messages.
There is a one-time registration fee due at time of enrollment equal to the monthly membership fee.
This fee may be discounted if multiple family members enroll at the same time.
